Are you using high density disks, by chance?

That would be a great first step in finding out what's wrong.

why type of disk should i be using

Double density.  It doesn't matter if it's labeled single or double sided, hard or soft sectored.  The key word is double density.  If your disks don't have hub rings - they're very likely high density.  If the media is also shiny, deep black - they're very likely high density.
