Documentation for SE/30 graphics card needed

Hello all,

One of my SE/30s came equipped with a graphics card (and even more (?)).
There is very little information on or about it. The two only parts of information is:
The PCB says:
MEMORY PLUS
COGAM-4 REV B
on it, and there is a EPROM, labeled:
Color SE30
A13 - IPT

I would to know more about this card, e.g. if there is any software for it, or documentation.
At least, I would like to know what I have here exactly 🙂

I successfully managed to get an output using a Mac/VGA DB15/HD15 adapter, but regardless what I set it to, 640x480 was the highest resolution I got. Maybe the card is not able to do more?

Any help/information is appreciated 🙂

640x480 is the max for this card.

I have one of these in my SE/30. 640x480 256. Works well with Mac OS 7.5-8.1.
I have not found software for it so I'm using it with standard Mac video drivers.

There was a post here in which members wanted to know what the writing on the 2 dies says, that is usually sanded off for some reason.

BTW, there is a control panel titled "Gamma" that helps improve the display quite a bit with this card. 256 shades of gray is possible on certain monitors.
