I have seen a couple of these show up in photos recently. They look really familiar but I can't put my finger on them. Is it a hard disk? What make + model?

It looks to me like a Multiple CD Disc Drive - you pull out a cartridge from inside and put the CDs in it and put it into the drive and you select any cd you need that you put in without swapping out disks.

Macdrone is correct, it is a power supply for early StyleWriters.

It's the only model which used this power supply. The StyleWriter II had a standard power cord, as did most of the other Canon-based StyleWriters (with some exceptions).
