What I have found is that my D34V-22 seems to be very sensitive to electrical noise. It's not possible to run it successfully without the caddie, which makes sorting out issues more challenging. However, once it's wrapped in its aluminium blanket it seems to be willing to play ball with the -A ROMs in my 128K, and System 2.0 appears to boot reliably.

My floppy is also a D34V-22 and is also sensitive to noise and only works with aluminum shield. The serial number on my drive is 943452 which is close to yours. AND, I also have Version A ROMs. I did notice in video that your IWM chip is Version A and mine is Version B. Do you know what month yours was manufactured? I have read so much about the incompatibility with Version A ROMs and this drive and yet mine works flawlessly everytime.
