According to this, most ADB mice have a pin on their chip for a second button.

He also gets a trackball from a Powerbook working as an external desktop device.
And builds a converter for Mac 128k mice to ADB from an old ADB mouse:
Plus mice put their interrupt wheel signals directly out on the DB9 port, and the chips in ADB mice are designed to convert interrupt wheel signals to ADB. | Posted by: CC_333 on 2016-02-26 10:37:09 This is a very neat upgrade, especially for those who might want to use such a mouse on a more modern Mac or PC via an ADB to USB converter (or an older PC with either an ADB to PS/2 or ADB to PC-Serial adapter), because most, if not all, modern OSes are very two-button mouse dependent (even OS X, although the option for a single button still exists).
