Looks like you needto build an additional adapter, dunno which settings you'll need. Played with what I could find at work last night,but haven't checked tha Apple pinout against my Mac Library.

I'm confused; this is a 13w3 adapter with switches, so... what is the host machine you're trying to hook a monitor to up with this?

I'm confused; this is a 13w3 adapter with switches . . . Makes absolute sense, the switches allow the user to set it up to run any of (most of?) the somewhat compatible flavors of 13W3 on machines listed on the left of the VGA colum in the spreadsheet I whipped up from info online at work last night.
The column to the right of VGA would be the same company that couldn't even manage to use the industry standard, specified connector for their half-@$$ed mis-implementation of the SCSI Spec.
Apparently, they opened up another can of FUBAR on their 13W3 mis-implementation. I gotta check GTttMFH, but it looks like they got Red & Blue on the wrong lines, one out of three ain't a bad batting average . . .
. . . or is it? ;D
Short version, a Mac specific 13W3 to VGA would be on my shopping list or I'd make a cable or adapter for the existing adapter. :-/

Well, I plugged the cable in anyway and I got a picture after the Supermac software cycled through on its own, I then hit the spacebar when the image came up. So I guess the Supermac card has the different signal types already, and just cycles through them, so the dip switches don't really matter with this card.

So before I got that LCD I messaged you about, I messaged the seller specifically about it not having dead pixels or any other issues. They assured me that it was in Grade A shape, which I took to mean that it's like-new. So I pulled the trigger and got it, especially with the free shipping, which is hard to do with a heavy monitor for so cheap.
So I went ahead and got it, but when it arrived, it had 4 dead pixels and makes a loud buzzing noise when plugged in. I would have to guess that the LCD's power supply is dying.
The seller refunded my money, but I'd 've rather had a working LCD. 🙁

Also, you may be able to bring those 'dead' pixels back to life. It depends on what kind of "dead" they are.
