I found such a card in an old machine, but there was no hard drive for me to get the drivers from. It says:
copyright 1996 CSI
P/N 00-9611-PCIS REV. 4.702
FCCID H39PCIS
and the sticker on the firmware ROM says:
LQS-PCI REV 1.50
Copyright CSI 1997
Anybody else have one of these? It looks like a decent card, and more hackable than my Keyspan.
