I built my own video adapter with separate sync and IDs set on a 21-inch multisync monitor. That was the problem. I'm now using a purchased adapter with the sync-on-green setting, and everything's fine now.
I figured it was something like that. I have a combination of passive adapters and ones with varying amounts of dip switches. I should probably replace them all with one of these at some point: https://www.bigmessowires.com/mac-sync-inator/ |

Seems like Interware already included the adapter for you. |
